Question:
No matter what exercises I do or way I hold my posture, I have a double chin. Which procedure(s) best rid a patient of a double chin?

Answer:
The treatment of a double chin will depend on several variables. The most common reason for a double chin to be apparent is the presence of excess fat in the submental region. In a young individual who has participated in a program of exercise and proper diet, liposuction or laser liposuction are ideal methods to address the excess fat of the double chin. The treatment is very well tolerated and the recovery is quite rapid.

Question:
I have the classic “turkey waddle” neck and I am ready to have it removed and reshaped. Where will the incisions be made so that they are as minimally visible as possible?

Answer:
Removing a “turkey gobbler” deformity from your neck is a very gratifying procedure because the changes are so dramatic. If the only issue you are concerned with is the excess tissue of the neck there are two approaches that can be performed in order to address the turkey gobbler deformity. In my experience, men are receptive to direct excision of the excess skin and muscle from the neck without performing a more extensive necklift or facelift procedure. The incision in this procedure is hidden in skin creases that run in horizontal fashion along the front of your neck. Women generally are not interested in this approach as the scars are more difficult to hide in females and therefor require insisions that go around the lower part of the ear into the hairline. These insisions are well camouflaged and give me good access to the relevant anatomy and enable me to perform the surgery successfully.

Question:
My lips are thin, pale, and barely there if I don’t wear lipstick. Are there any injectables that will plump up my lips as well as improve my lip color so they are a more defining feature?

Answer:
There are several injectable available to plump up your lips. I prefer to stay with the hyaluronic acid fillers such as, Restylane and Juvederm to perform these procedures. I would caution you that the color of your lips will and should not change with the use of dermal fillers. I am not really a fan of tattooing of the lips as your lips will change as you age and the tattooing will not, but this is the only non-surgical way to permanently change the color of your lips.

Question:
I have very sunken looking cheeks. Are fat injections a good solution for this?

Answer:
Cheek implants are not a one size fits all situation. There are several varieties of cheek implants that are of many sizes and shapes. After a thorough evaluation of your face we will be able to determine the best implant for you if that is what is necessary. I have had a great deal of success with fat injections to the face in the areas that I have place cheek implants in the past. The fat is harvested from your hips or abdomen and once it is injected into your cheeks will give you a natural appearing face that will also feel natural. I would be happy to discuss the pros and cons of each procedure with you.

Question:
My hands are aging faster than the rest of my body. My nickname is granny hands and while it didn’t bother me at first, I am becoming self-conscious. Can Botox injections be used to smooth wrinkles other than those in the face?

Answer:
It is interesting that you and your friends have noticed the significant aging of your hands. I feel that other than the face, the appearance of the hands and the neck really disclose the patients age. Botox is not really used in the hands for wrinkles but can be used in the palm of the hands for hyperhydrosis or excessive sweating. For aging and wrinkled hands I prefer to use dermal fillers. Small amounts of dermal fillers such as Restylane, Juvederm, or Radiessse are injected into the back of the hands and massaged in between the veins and tendons that are so prominent and which give the hands the appearance of aging. The dermal filler looks incredibly natural and the procedure is extremely well tolerated.

Question:
My pregnancy has been a blessed and beautiful time, but I am looking forward to getting back in shape. One problem that I am worried about reversing all the stretch marks that have formed on my belly. Can fat injections be used to smooth out my stretch marks?

Answer:
Stretch marks are one of the more common complaints that we are seeing in our office. Stretch marks represent a change in the actual skin where the stretch marks have appeared. Fat injections have not shown significant benefit in the improvement of stretch marks. What has shown real promise is the use of the fractionated erbium laser. In our practice we use the Fraxel laser for this purpose. The Fraxel laser requires approximately 4-5 treatments and our patients have been very pleased with the reduction in the appearance of their stretch marks.

Question:
My skin is fair and several blue veins are starting to show through quite prominently. Will laser skin resurfacing reduce the appearance of the blue lines even though my skin is extremely fair?

Answer:
The blue lines that you are seeing through your thin skin are veins. Laser skin resurfacing uses laser energy to heat the skin and enable new collagen to form which leads to tightened and healthier skin. The laser energy is designed to only penetrate as deeply as the outer most layer of the skin into what is known as the dermis. The laser energy should not go any deeper than the dermis. The veins that you are seeing in your skin are actually deeper than the laser will penetrate and the appearance of these veins should not be affected with laser resurfacing. Veins also vary in their size. The lasers that are designed for skin resurfacing are not designed to treat veins. The energy wavelength needed for skin resurfacing is very different than the energy that should be applied to your skin to treat veins in the deeper tissue.

Question:
I have a mole on my arm that I would like removed as soon as possible because of some family history with cancerous moles. I have had moles removed using a freezing method, but I was left with a scar. Will laser skin resurfacing used to remove a mole leave a scar?

Answer:
If you have a mole on your arm and are concerned with a family history of skin cancer, the best approach to address this mole would be surgical excision. Laser would be an alternative to surgical excision but only if the laser is used as a cutting tool to remove the skin and not destroy the skin prior to evaluating the lesion for any pathological changes. In this scenario, laser is likely not going to be of any increased benefit with regards to scarring compared to traditional meticulous surgical techniques.
