Question: Looking at before and after photos of rhinoplasty patients, I feel as if all of their noses look the same. I only want a very subtle change in my nose, and I don’t want it to look like everyone else’s. Would you be able to this?

Answer: Rhinoplasty surgery is not a one size fits all endeavor.; I have also seen photos of rhinoplasty results of various surgeons whose noses really do all look the same. I do not feel that noses should look alike.; The nose should fit the face of the individual who is seeking rhinoplastic surgery, not the other way around. I spend a great deal of time with my patients so that we can discuss what nose would look best on their face.; I employ the use of computer imaging to help demonstrate the changes to the nose that the patient should expect from rhinoplasty surgery.; I emphasize that I can only create a nose that the patients anatomy will let me create.; In other words, there are limits to what can be done with a nose. I am certainly willing to have a look at photos of noses that a patient may bring to their rhinoplasty consultation so that I get a sense of what the patient is visualizing in their nose, however, I am careful in setting realistic expectations for the patient and what they can expect from their surgical procedure.

Question :
I feel like my nose droops down, not the tip exactly but more the skin that is between the nostrils. It almost seems like the skin there is too fat and needs to be leveled off. Can this be done?

Answer :
Although it appears that it is the skin that seems to be drooping in the area between the droop. Often times the droopy appearance of your nose between your nostrils is a combination of issues such as excess cartilage, excess fat, and laxity of the soft tissue between your nostrils. During your rhinoplasty consultation we will evaluate this area and we can discuss which issues seem to be contributing the most to your appearance.

Question :
When your own tissue is used to augment your nose during rhinoplasty, is there a chance that this would be reabsorbed by the body over time and the augmentation would fade?

Answer :
I prefer to use your own tissue to augment your nose during rhinoplasty. I will generally use septal cartilage or auricular cartilage to augment your nose. I have been extremely pleased with the rhinoplasty results I am able to obtain with autologous tissue and one of the benefits of using your own tissue is that the tissue will not resorb or disappear over time which would obviously affect your rhinoplasty result.

Question :
In what ways is septoplasty different from rhinoplasty?

Answer :
Septoplasty surgery is used to correct a deviated nasal septum.  The nasal septum is the bony and cartilaginous structure that exists inside your nose that separates the nose into two sides.  A deviated septum is responsible for nasal congestion, recurrent sinusitis, and occasionally headaches.  Rhinoplasty is the surgical procedure that addresses the aesthetic issues related to your nose and not the functional issues.  External changes to the nose is considered rhinoplasty.

Question :
I’m a Middle Eastern man and I don’t like my profile. Is there a type of nose job I can get that would cater to my specific needs?

Answer :
I happen to perform many rhinoplasty surgeries on patients of middle eastern decent.  Partially because I practice in Washington, DC, and in part because of referrals from patients and physicians who are also of Middle Eastern decent.  I feel that each nose I treat needs to be individualized to the specific requirements of the patient.  It is not to say that two people cannot look the same, but, rhinoplasty is an art where the goal is to create a nose that looks best on the particular patient you are treating.  I can certainly help create a nose that is appropriate for you.

Question :
I’ve become a recluse ever since my rhinoplasty surgery. I hate my nose and I’m so embarrassed that I never go out now or see friends. With revision rhinoplasty, what is the likelihood that the nose will be what you originally wanted or at least significantly improved?

Answer :
Too be clear, I did not perform your original rhinoplasty.  I can however, totally sympathize with how you may feel about your nose.  I would like to assure you that after 20 years of revision rhinoplasty surgery you are probably being much more critical of your nose than others are.  Having said that, although I have not yet seen the condition of your nose following your rhinoplasty, I am certain that I can create a nose that you will be significantly more pleased with than the nose that you received from your rhinoplasty if it is in fact, that bad.  The question is really whether or not I can create the nose that you had originally wanted from your rhinoplasty.  Although I am confident in my ability to create a pleasing nose, the surgeon that you had seen may have promised you more than anyone can deliver from rhinoplasty surgery and we will have to discuss realistic rhinoplasty expectations.  I do know that I can restore your self confidence if your rhinoplasty result is what has damaged it so significantly.

Question :
I’m African American and I’ve heard that my ethnicity can limit the amount of refinement that I can expect in my nasal tip. Why is this?

Answer :
I would agree with the statement that you have made for the following reasons.  Ethnic and more specifically African American noses tend to have thicker nasal tip skin, more subcutaneous nasal tip fat, and weaker nasal tip cartilages.  The thickness of the nasal tip skin is probably the biggest factor in the limitation of tip refinement in African American rhinoplasty.  It is not to say that nasal tip enhancement cannot take place, as I have extremely happy African American rhinoplasty patients who have dramatic improvement in the nasal tip appearance, but, over promising and under delivering on the post rhinoplasty appearance of the nasal tip is only going to lead to unhappy patients.

Question :
Do all doctors take preoperative rhinoplasty photos and why?

Answer :
I cannot speak for all doctors, but, I take preoperative rhinoplasty photos on everyone.  I use these photos during my rhinoplasty consultation with the patient and more importantly, I use these photos for rhinoplasty planning.  Following rhinoplasty surgery, it is always a good idea to have the preoperative photos as patients often like to compare their pre and post operative results.  I also use these images when permission is given to enhance my web site, during rhinoplasty lectures to other physicians, and for my own learning purposes.
