<p><a href="/ser_dermal.html"><img style="float:right; padding-left:10px" src="/images/renudc-blog-post-16-04-13.jpg" width="280"></a><strong>Answer: </strong><a href="/browlift.html">Aging of the forehead</a> is  managed in a variety of ways depending upon what the issues are that are contributing to the appearance of an aging forehead. In cases where there are hyperdynamic lines of facial expression, such as in the area between your eyes and around your eyes, Botox and Dysport are the best choices. When properly injected, <a href="/ser_botox.html">Botox</a> and Dysport will soften these lines and prevent these lines from becoming more pronounced. Neuromodulators like Botox and Dysport will not decrease fine and deep lines in the skin of the forehead and in cases where the brow has fallen a considerable amount, surgery to elevate the eyebrow will be necessary to adequately rejuvenate the forehead.</p>

<p><strong>Answer: </strong>The decision to make an incision on the inside or the  outside of the eyelid pertains to <a href="/blepharoplasty.html">lower lid blepharoplasty</a>.  In upper eyelid surgery, if there is excess skin, which is generally the case  for upper eyelid surgery, then the incision is made to remove excess skin and  therefor the incision is done on the outside of the eyelid.</p>

<p>In <a href="/blog/category/blepharoplastyeyelid-surgery">lower  eyelid surgery</a>, the decision to make an incision on the inside  versus the outside depends upon a few different factors. The status of  the skin, muscle, and to some extent the strength and tone of the lower eyelid  will guide our decision to perform an external incision or not. The age of the patient and the status of the fat or bags of the lower eyelids will also be taken into consideration. Generally, in younger patients with very little  excess skin, lower eyelid surgery can be performed without the removal of any lower eyelid skin and therefor the incision can be make on the inside of the eyelid. In older individuals, there is generally a combination of too much fat, excess skin and muscle, and laxity of the lower eyelid which will all need to be addressed to properly rejuvenate the lower eyelid and will require an external incision to be made.</p>

<p><strong>Answer: </strong>There are a few different approaches to otoplasty. The <a href="/otoplasty.html">otoplasty procedure</a> that will work best for any individual depends on a few different  variables. The age of the patient and the overall shape and size of the ear, just to name a few. When a patient is very young, cartilage abrasion through small incisions in the ear and a proper fitting dressing may be all that is needed to correct the shape of the ear. Most patients require  more than that.</p>
<p>There are a few schools of thought in how to best  correct the <a href="/blog/category/otoplasty">shape of the ear</a>. There are those who use cartilage cutting techniques, and those who use cartilage folding and suturing techniques. Sometimes a combination of the two techniques is necessary in very prominent ears. I tend to favor cartilage folding techniques. During the otoplasty procedure the poorly  formed anti helix is created in the pinna using a Mustarde technique. If after the normal contour of the ear is corrected the ear still is sticking out, then I use a Furnass setback suture in order to bring the ear closer to the head on front and back views. I like the cartilage sculpting and folding techniques because they are reversible in the early stages and adjustable. More importantly, they leave the ear looking natural without any of the sharp transitions in the ear architecture that is so apparent in the cartilage cutting technique.</p>

If an accumulation of blood or fluid in the cartilage of the ear should occur following an otoplasty procedure and goes unrecognized for a period of time, then theoretically a cauliflower ear can result from the <a href="/blog/category/otoplasty">otoplasty procedure</a>. Cauliflower ear occurs when an auricle is damaged and blood or fluid accumulate between the cartilage of the ear and the blood supply to the ear, resulting in cartilage death and eventually disfigurement.</p>
